The LHIN (Local Health Integration Network) is a central agency that creates and manages all applications for every long term care home in Ontario. A phone call to the LHIN is all that is needed to initiate an application to move into our Home.
A caseworker will then schedule a time to come and visit you and/or your loved one in person, whether you are in your own home or in hospital. The case worker will take care of completing an application package and sending it to the Homes of your choosing.
Every community in Ontario has a local branch of the LHIN. To find the LHIN that serves your area, follow this link and select your region: http://healthcareathome.ca/
The Ministry of Health and Long-Term Care sets the rates for all long-term care homes in Ontario. These rates are set every year and cover a variety of services, including nursing and personal care; meals and snacks; assistive devices to encourage independence; on-site medical care provided by a physician who makes regular visits to the home, as well as housekeeping and laundry services. Accommodation is a separate fee. Additionally, optional services, such as cable TV, hair salon appointments, dentist appointments, vision care or specialized foot care, can also be charged to the resident separately.
The Home welcomes families to visit as often as possible and to stay as long as possible. Guests are a very important part of a resident’s life, and we encourage regular visits by relatives and friends. We practice an open hour’s visitation policy with consideration for the comfort and schedules of all residents. It is suggested that visitors who plan to visit after 8:30 p.m. call the Home and advise the Charge Nurse of their plans in advance.
You can expect to receive the following services in a long-term care home:
Optional services are usually available for a fee such as hairdressing, cable TV, telephone services, and transportation.
